Singe Point of Access/Accountability (SPOA)

SPOA is a New York State Office of Mental Health (OMH) regulated initiative.  UCDMH is tasked with facilitating timely access and linkage to an array of intensive OMH services for high risk adults, children/adolescents and families.  SPOA services are designed to promote recovery and are personally tailored and responsive to individual needs and preferences. Child SPOA Application

This application is used to request one of the following CSPOA services:

  • OMH Home and Community Based Waiver Services

  • OMH Non Medicaid Care Coordination

  • OMH Family Peer Support

  • OMH Residential Treatment Facility

  • OMH Community Residence

Adult SPOA Application for Care Management/Care Coordination/ACT

This application is used to request one of the following SPOA services

  • Assertive Community Treatment (ACT)

  • Mental Heath Association Care Management Health Home (MHA CMHH)

  • Office of Mental Health Care Coordination (OMH CC)

  • Family of Woodstock and Mental Health Association Transitions Care Management

  • Family of Woodstock Getting Ahead Program Care Management (GAP CM)

  • State Intensive Care Management (ICM)

SPOA Adult Resident Services Application

This application is used to request one of the following residential programs:

Gateway Community Industries: Level I Community Residence, Level II Supportive Apartments, Level III Supported Housing

Rehabilitation Support Services, Inc. (RSS): Level I Community Residence, Level II Supportive Apartments, Level III Supported Housing

The Mental Health Association in Ulster County, Inc. (MHA): Level II Supportive Apartments, Level III Supported Housing
