SINGLE POINT OF ACCESS/ACCOUNTABILITY (SPOA)

 

SPOA is a New York State Office of Mental Health (OMH) regulated initiative.  UCDMH is tasked with facilitating timely access and linkage to an array of intensive OMH services for high-risk adults, children/adolescents, and families.  SPOA services are designed to promote recovery and are personally tailored and responsive to individual needs and preferences. The following are links to SPOA information and applications:

Child SPOA Application

This application is used to request one of the following CSPOA services:

  • OMH Home and Community-Based Waiver Services

  • OMH Non Medicaid Care Coordination

  • OMH Family Peer Support

  • OMH Residential Treatment Facility

  • OMH Community Residence

Adult SPOA Application for Care Management/Care Coordination/ACT

This application is used to request one of the following SPOA services

  • Assertive Community Treatment (ACT)

  • Mental Heath Association Care Management Health Home (MHA CMHH)

  • Office of Mental Health Care Coordination (OMH CC)

  • Family of Woodstock and Mental Health Association Transitions Care Management

  • Family of Woodstock Getting Ahead Program Care Management (GAP CM)

  • State Intensive Care Management (ICM)

SPOA Adult Resident Services Application

This application is used to request one of the following residential programs:

Gateway Community Industries: Level I Community Residence, Level II Supportive Apartments, Level III Supported Housing

Rehabilitation Support Services, Inc. (RSS): Level I Community Residence, Level II Supportive Apartments, Level III Supported Housing

The Mental Health Association in Ulster County, Inc. (MHA): Level II Supportive Apartments, Level III Supported Housing

People USA: Level III Supported Housing
