MENTAL HEALTH CASE MANAGEMENT AND CARE COORDINATION

 

WHAT IS CARE MANAGEMENT AND CARE COORDINATION? Care management and care coordination is a direct resource for adults 18 years of age and up. An individual care manager or care coordinator provides services in the home and the community. Case management has varying levels of intensity depending on the program. The goals of care management and care coordination are to strengthen and empower adults so that they can live safe and productive lives. Care management and care coordination services include ongoing assessment of the adult’s strengths and challenges; development of an adult/person-centered; goal oriented service plan; linking the adult to appropriate services; monitoring effectiveness of services as they relate to the service plan; and advocacy.

WHAT IS THE ELIGIBILITY CRITERIA? The consumer must possess a DSM-5 diagnosis that meets criteria for Serious Mental Illness (SMI), or a DSM-5 diagnosis with extended impairment of functioning due to mental illness.

WHAT IS ACT? Assertive Community Treatment, or ACT, is an evidenced-based practice that offers treatment, rehabilitation and support services, using a person-centered, recovery-based approach, to individuals that have been diagnosed with a severe and persistent mental illness. ACT services are provided to individuals by a mobile, multi-disciplinary team in community settings.

The Mental Health Association in Ulster County, INc. (MHA)

MHA offers Health Homes, which is a care coordination service available to individuals who are on Medicaid and have two qualifying medical health diagnoses or a mental health diagnosis. Health Home care coordination services aim to address the person as a whole. Service areas include medical health, mental health, housing, finances, vocational services, social and recreational support services, substance abuse, legal services, and any other area of need. Care Managers link and refer clients to relevant support services, communicate and coordinate with other service providers, advocate for client services, and provide education and support to clients.

Family of Woodstock

P.R.O.S. Program

PROS is a comprehensive, psychiatric rehabilitation service that supports people in their journey towards wellness and recovery. These services offer individualized treatment and support in our Center for Rehabilitation, Education, Advocacy, Treatment and Employment.

The Center offers an array of person-centered, goal-oriented services designed to assist individuals in:

  • Managing symptoms

  • Developing the skills and accessing the supports needed to be successful and satisfied in chosen life roles

  • Honoring the process of recovery

PROS individual sessions and small groups are offered Monday through Friday from 9:00 a.m. to 3:00 p.m. Services are provided by a team of licensed psychiatrists, nurses, clinical social workers, rehabilitation counselors, group facilitators, and alcohol & substance abuse counselors.

Adult Behavioral Health Home and Community Based Services (BH HCBS)

Adult BH HCBS can help you achieve your life goals and be more involved in your community. These services can help increase your independence, support your education and employment goals, provide peer support, and help manage crises. BH HCBS are available for people 21 and over who are enrolled in a Medicaid Managed Care Health and Recovery Plan (HARP) and found eligible after completing the NYS Eligibility Assessment.
